Safety Precautions

  • Always check for allergies (latex, adhesive, iodine)
  • Use proper gauge for intended therapy — 18G for blood, 20-22G for most infusions
  • Never insert IV in an extremity with a fistula, mastectomy side, or existing infiltration
  • Activate catheter safety mechanism immediately after insertion
  • Maximum of 2 attempts per nurse — then get another clinician
  • IV sites must be assessed and documented every shift minimum
